

Most insurance plans require a referral from your primary care physician prior to your evaluation. Some plans require pre-authorization, co-payments, network providers, deductibles or other provisions. Please contact your plan's patient assistance department if you are uncertain about your coverage or how to be referred to Audioscope.

Audioscope will be happy to file insurance forms or assist you in completing paperwork for reimbursement. Please be prepared to make co-payment, if applicable, on the date of your visit. If you have not yet met your deductible or if you are a self-pay patient you should also be prepared to pay for your office visit on the day of service.
Hearing aid purchases require a deposit on the date of the order of 1/2 of the total cost and balance of payment at the end of a successful 30-day trial period. A complete refund of all money paid is made if the devices are returned within the 30-day period in compliance with California state law (Beverly-Song Act). Earmolds require full payment at the time of order and are nonrefundable.
